You have the right to privacy during the delivery of services and to have your personal and health information protected as required by applicable law.

Your Responsibilities as a Patient

As a patient, you are responsible for:
Providing Accurate Information
Provide accurate and complete information about your health history, medications, allergies, symptoms, and other information relevant to your care.
Communicating Changes in Your Condition
Tell your healthcare provider or A Flourishing Life Health Care team about significant changes in your condition, symptoms, medications, or other concerns.
Following Your Plan of Care
Participate in your plan of care and follow agreed-upon instructions to the best of your ability.
Taking Medications as Directed
Take medications as prescribed and communicate questions, concerns, side effects, or difficulties with your healthcare provider.
Keeping Appointments
Make reasonable efforts to keep scheduled visits and appointments. Notify the agency as soon as possible if you need to cancel or reschedule.
Providing a Safe Environment
Help maintain a safe environment for healthcare professionals providing services in your home.
Treating Staff With Respect
Treat A Flourishing Life Health Care employees, contractors, and healthcare professionals with courtesy and respect.
Threatening, abusive, discriminatory, or unsafe behavior is not acceptable.
Asking Questions
Ask questions when you do not understand your care, medications, instructions, or other information provided by your healthcare team.
Participating in Your Care
Participate in decisions regarding your care and communicate your goals, preferences, concerns, and limitations.
Providing Updated Information
Notify A Flourishing Life Health Care of changes to your address, telephone number, emergency contact, insurance information, physician, or other information that may affect your care.
Meeting Financial Responsibilities
Provide accurate insurance and payment information and fulfill applicable financial responsibilities related to services received.
Your Right to Make a Complaint or File a Grievance
